Cellulite can affect anyone, whether one is slim or overweight. It is not a problem of excess fat. Rather it is the perfect storm of skin elasticity loss, tight fibrous septae and the fat being able to bulge through the less elastic skin. We believe Qwo will be a great intervention for cellulite. We feel it will work even better when combined with a skin tightening procedure such as Exilis Ultra, Thermage or Ultherapy. <div><br /><div>Qwo is an injectable treatment for cellulite using collagenase clostridium histolyticum. It breaks down collagen types 1 and 3 which are found in the fibrous septae between pockets of fat cells. These fibrous septae shorten with time, often in an irregular manner. If the septae weaken, the fat can bulge through and create a cottage cheese type of appearance on the skin. The skin itself also looses elasticity with time. Collagen and elastin fibers deteriorate as we age (sadly, as we know). This less elastic skin is more strongly affected and pulled at various strengths by the actions of the tight fibrous septae. </div><div><br /></div><div>Our current treatments for cellulite include Velashape, Exilis Ultra, Vasersmooth, Cellulaze, Z wave, and fillers such as Bellafill or Sculptra. Velashape is a multi-modality platform which utilizes radiofrequency energy and infrared energy to heat and induce new collagen and elastin fiber production in the skin while also being able to reduce bulges of fat. The concurrent mechanical suction and tissue manipulation allows the fibrous septae to be gently stretched and helps to improve vascularity and health to the tissues. Exilis Ultra uses monopolar radiofrequency to improve skin elasticity and also to permanently reduce the number of fat cells in the fat globules. Vasersmooth and Cellulaze are minimally invasive treatments using a small probe that travels under the skin to cut through the fibrous septae. Vasersmooth additionally can tackle and disrupt the fat globules. Z wave uses photoacoustic energy and is a great standalong procedure but even more effective when preceded by any of the prior procedures. The sound waves improve vascular health and help to further break down fat cells and tenderize the fibrous septae.</div><div><br /></div><div>The injectables such as Bellafill or Sculptra are great for indentions in the skin. These indentions can be filled out immediately and give a smooth appearance that is long lasting. </div><div><br /></div><div>We do have injectables that break down fat as well, specifically knows as Kybella, or Deoxycholic Acid. Kybella works great on dissolving fat cell membranes but does not dissolve type 1 or 3 collagen. </div><div><br /></div><div>Qwo is the first injectable of its kind that essentially replaces Vasersmooth or Cellulaze in terms of being able to break the fibrous septae that pull the skin down. <br />Most of you have heard of coolsculpting by now. It has almost become a household name when one thinks of wanting to get rid of unwanted areas of fat without downtime. Coolsculpting, as the name implies, works by lowering the temperature in your subcutaneous (under the skin) tissues where the fat exists to a temperature that is found to push fat cells into apoptosis (a process known as programmed cell death where the cell is pushed into the process of dying off naturally). The fat that is slowly released from the dying fat cells is eliminated via your lymphatic system. There is no danger to your health. It does not cause clogging of your vessels or a spike in your blood serum levels of fat or cholesterol. Think of it more a like a bad bruise which your body heals naturally. Coolsculpting was developed from an observation that children who held cold lollipops in their cheeks for extended time periods developed dimples as a result of cheek fat loss. Just about any body area can be treated with the various sized suction and nonsuction applicators available. The most common areas treated are the stomach, love handles, arms, inner and outer thighs, and the chin. Bra fat, knees, calves, banana rolls, male breasts (gynecomastia) can also be greatly improved with coolsculpting. Generally, one can expect about 24-25% fat loss in a single treatment, with results seen within 6-12 weeks of treatment. We recommend 2-3 treatments to the same area for best results. Though not designed for skin tightening, if the skin has decent elasticity, one is generally not left with loose hanging skin.<br />

So then what is warmsculpting? Warmsculpting is becoming increasingly more popular. It generally refers to a competing technology called Sculpsure, which uses at 1060 nm wavelength laser to target subcutaneous fat. The laser causes damage to the fat cell wall. It also gives some gentle heating to the elastic fibers and collagen fibers in the skin, inducing a bit of skin tightening, which is an advantage to coolsculpting. This procedure is termed warmsculpting because one feels heat vs cold. the procedure takes 25 minutes, vs the 35 min-1 hour 15 min cycles of coolsculpting. Additionally, both love handles can usually be treated in one single 25 minute treatment compared to two cycles of coolsculpting requiring 1 hour and 10 mins of treatment time.<br />

Because we have both technologies, we can help you chose which procedure would be best for your particular case. We have other noninvasive tools also such as Vanquish ME (excellent for generally larger bellies and love handles, inner and outer thighs), and Emsculpt for fat loss and muscle gain as well as vaser liposuction for the quickest and best results. Many times, we might recommend a combination of Coolsculpting and Warmsculpting in alternating treatments to achieve the best results. Warmsculpting is a bit more versatile with where it can be placed and is especially useful in areas of less pinchable fat where the suction applicators of coolsculpting might fail.<br />

The reason we use dermal fillers is because as we age, we naturally loose some of our youthful features including fat pockets which either shrink or shift and hence we loose that soft cushioning under our skin which reflects light in a positive manner off our face to give us a healthy glow. Of course the collagen and elastin fibers also reduce with age, causing us to loose that lush, soft hydrated glow. T<span style="font-size: 12.8px;">here is also structural bone loss that occurs and contributes to undereye and cheek hollowing. Fillers can help correct these problems.</span></div>

The common areas for volume loss include the temples, lateral cheeks, central cheek, undereyes, chin and the lips. Many patients come to the office with concerns of the nasolabial folds or laugh lines looking deepened and while we may at times inject fillers directly into those lines, a more natural appearing approach is to lift the skin upwards by restoring volume in the midface, lateral cheeks and temples. Adding volume along the jawline also gives amazing and beautiful results as does adding volume to the chin area to soften the jowls. </div>

When a patient presents with more significant volume loss, it's a great idea to build up collagen using Sculptra and Bellafill, both of which give a more long lasting improvement and stimulate your own skin to make new collagen. Bellafill is also FDA approved for acne scars and performs very well. Both Sculptra and Bellafill lay down a matrix upon which we can build with other fillers if needed. It does often take a combination of fillers to achieve the best results. The fillers don't have to be injected all at once either. We can space the treatments out a bit and approach the volume loss stepwise.</div>

The most popular injectables for patients of all ages are the hyaluronic acid fillers such as Voluma, Juvederm and Restylane of which there are several varieties. Some are more hydrophilic, meaning they attract more water and others are a bit thicker and stickier for areas such as the cheeks. We also like to use Radiesse which is a calcium based white product that also helps to stimulate your own collagen growth. The hyaluronic acid fillers provide excellent volume but do not stimulate your own collagen. Even though fillers do gradually diminish over time, they don't completely go away all the way and subsequent treatments build nicely upon the prior treatments. </div>

spread of the botulinum toxin into neighboring muscles can occur

shows sadness. But even the most feared symptoms dissipate quickly as Botox is not permanent and breaks down over just 3 months. There are currently three brand names of Neurotoxins: Botox Cosmetic, Dysport and Xeomin. In our practice, we use Dysport and Botox Cosmetic primarily as they give good long lasting improvements. We also keep Xeomin for patients who may be allergic to egg products. </span></div>

In a small number of cases, fillers can also move slightly away from the line or wrinkle itself with movement of the line. I recommend using fillers in conjunction with Botox for best results if the lines are very deep.</span></span></div>

procedures which are fairly similar referred to as No-Tox as well. They are known as GFX treatment (Glabellar Frown Treatment) and ThermiRase.
These treatments utilize a small radiofrequency probe directed at the
nerve fibers which innervate the glabellar complex of muscles as they arise
from the temples. Both procedures work beautifully for the frown lines in between the brows for an immediate and long lasting to potentially permanent improvement of the frown lines. If 2 cm of the nerve is treated, generally the treatment can be expected to be permanent. We have successfully helped many of our patients to never require Botox in their frown lines again. However, the shortcoming here is that this procedure really only works on the frown lines between the brows sometimes referred to as the 11's. GFX and ThermiRase are not recommended for forehead lines or crow's feet. One other interested application however is for neck bands, though clear protocols have not yet been established for this area. </span></div>

Studies have shown that if Botox is injected around a fresh sutured incision, that the incision heals with significantly less scarring? It is believed this is because the Botox eliminates tension and disruptions to the healing wound which allows it to heal more smoothly. The scars are visibly less apparent when treated with Botox!<br />

Botox is also great for pucker lines around the mouth. It does prevent a strong pucker, but in turns helps the lines smooth out. Also, if placed correctly, can give a nice little flip upwards of the lip which gives the appearance of a fuller lip without a dermal filler.<br />

Botox is awesome for hyperhidrosis or excessive sweating. When placed superficially under the skin it blocks the sweat glands and hence, no more sweating. The most common areas treated are underarms, hands and forehead or scalp. Some patients are even requesting a "blowtox" as it allows patients not to sweat from their scalp after a workout and saves a hairwash afterwards! A single treatment can last from 3 months to a year, partly dependent on the number of units of Botox used. A few of our patients have noticed marked reduction in sweating longterm after several treatments and now require very few visits if any. We do also offer ThermiDry which is a one or two time procedure which also dramatically reduces the sweat glands and odor longterm.<br />

Finally, Botox can be useful in Acne. In dilute amounts, it can be carefully injected in the more acne prone areas to reduce sebum production and helps the skin heal with significantly less scarring. The skin of course also appears more healthy, smooth and glowing wherever the Botox is injected.<br />

It presents with frequent tearing, redness and irritation of the eyelids. The most common treatment is oral antibiotics such as Doxycycline or Minocycline. Patients find variable response rates to these medicines and often don't like the idea of taking antibiotics long term. <br />

In our practice, we have treated numerous patients with ocular rosacea using the VBeam laser. VBeam is a pulsed dye laser, emitting a 595 nm wavelength light beam which beautifully targets surface blood vessels. The VBeam has a special linear spot treatment, which along with cryogen (blast of ice) pre and post pulse cooling, allows for effective and safe treatment of the eyelid margins without burning or bruising. We also use a circular larger spot size for treatment of surface vessels on the cheeks, chin, nose and forehead for facial rosacea. Patients usually respond even after a single treatment though we suggest a series of 3-5 initial treatments to gain control of the problem followed by once a quarter treatments for maintenance. <br />

Ocular rosacea patients who have received VBeam treatments from us have been able to discontinue their antibiotics and topical rosacea creams and enjoy an increased quality of life with less embarrassment due to red, watery eyes or flushing cheeks.<br />

The VBeam laser, though now often replaced by Intense Pulse Light machines such as BBL and other IPL's, continues to be one of our most favorite lasers for the superior and results it provides and the diversity in which it can be used.<br />

For certain skin problems there may be a one treatment, one procedure fix it all solution. Very often though, I find that the patient's true concern will need a medley of a few procedures to solve the problems that are contributing to the concern. For example, a patient may come in with complaints of the parantheses (lines around the mouth) or nasolabial folds being too deep. They have read that these lines can be filled with a filler. Yes, perfect. In many cases, just adding a bit of strategic filler directly into the folds will solve the problem. In some cases, however, the volume loss that is contributing to the folds may be closer to the temples, upper cheeks, tear troughs and jawline. Restoring volume via filler to these areas can give a very natural, soft, and more dramatic improvement to the original problem as the filler places the skin back into position. Often accompanying this loss of volume in the face is also the fact that the skin is loosing collagen. Under normal circumstances, we loose about 1% of our collagen content per year. Genetics and lifestyle contribute to various degrees of deterioration over time. The loss of collagen makes the skin less elastic, thinner and weaker. Luckily, we have ways to combat this collagen loss with various procedures which heat the skin and force the skin to make new tightly woven collagen fibers. If we start stimulating the collagen at the earliest signs of loss, it can go a long way in keeping our skin young as we age. So, the patient with the nasolabial fold concern may not be happy with just fillers, but may also need some tightening of the skin to achieve the best, most natural and dramatic results. Procedures such as Skin Tyte, Thermage, Exilis Elite, ThermiRF and Ultherapy can all be used depending on what degree of tightening is required and the patient's budget. Of the tightening procedures, ThermiRF and Ultherapy win. Or the patient may need a skin resurfacing procedure, if along with the loosening of the skin, there is also sun damage and wrinkling involved. The company has done a phenomenal job marketing their procedure for noninvasive body sculpting. The idea comes from world renowned laser guru, Dr. Rox Anderson, of the Massachusetts General Hospital and Harvard Medical School. By using cold temperatures to freeze the fat cells, the fat cells undergo apoptosis and die off. It takes about 3 months for the body to slowly eliminate all the dead fat cells from your body. About 24% of the fat cells are killed in one 40-60 minute session. The fat must be pinchable and must fit into a predesigned applicator for the procedure to work. If you are having upper and lower stomach and love handles treated, you are looking at 4 hours of treatment time. Three sessions are recommended for best results, spaced from 6-12 weeks apart. After a coolsculpting procedure, the skin has to initially thaw out. Gentle massaging can be helpful here. Many patients feel uncomfortable after the procedure and describe pins and needles types of sensations. The pain can continue or worsen over the next few weeks to months as the nerves are often affected. There have also been cases of the fat deposits actually growing rather than shrinking, now well documented as Paradoxical Adipose Hyperplasia. Finally, cryolipolysis does not offer any direct skin tightening benefits, though some clinicians are reporting seeing tightening of the skin. I feel it is definitely a procedure that works and the aggressive marketing has made it a household name. Is it the best procedure available for noninvasive body sculpting? While it's a good option, we feel there are even better options.<br />

One procedure we absolutely love is Vanquish. Vanquish is completely painless. It uses focused radiofrequency currents that are driven through the fat cells, causing them to heat up to 45 degrees celsius. At this temperature, the fat cells undergo apoptosis or controlled cell death and die off. They are slowly eliminated from the body over 12 weeks time. The fat does not need to be pinchable. The procedure can be performed on slim to very heavy patients and BMI is not as critical in evaluating candidacy as it is with Coolsculpting. A large area is treated over 30-45 minutes from the stomach to love handles. It is recommended that the patient return weekly for 4-6 sessions. Besides the stomach and love handles, other areas that can be treated include back fat, arms, and thighs. For dramatic reductions of fat cells of up to 60-80% after 4-6 sessions, Vanquish is by far the most effective. It also has no associated post treatment pain. All normal activities can be resumed immediately. No special diets are required. We do, however, request that patients consume 8 glasses of water with any noninvasive body sculpting procedure to help flush the fat out. There is also no danger of Paradoxical Adipose Hyperplasia as there is with Coolsculpting. In our practice, we have seen some very dramatic results with Vanquish. The skin usually also heats up to 42 degrees celsius during the procedure providing the added benefit of well documented collagen stimulation and hence true body contouring and skin tightening.<br />

New to the market is a procedure called Sculpsure. Although we love Vanquish, we feel Sculpsure has a good position for treating specific pockets of fat. It can also treat larger areas. The advantage is that it does not require weekly visits and can be accomplished in a single 25 minute session per area. An area consists of 4 applicators that can be placed anywhere for that 25 minute session. To treat an upper and lower abdomen and love handles takes 1-1.5 hours (compared to the 4 hours it would take with Coolsculpting). One can expect to see at least a 25% reduction in fat cells after a single session. Patients may need an additional 1-2 sessions spaced 6-12 weeks apart. Unlike Coolsculpting, Sculpsure does not require the fat to be pinchable. It can be administered on any part of the body except the chin (for now). There is absolutely no recovery time or post treatment nerve pain. One does not have to wait for the skin to thaw out as with Coolsculpting and there is no risk of Paradoxical Adipose Hyperplasia. Also, heating is great for stimulating new collagen growth in the skin. For all these reasons, most practices on the east and west coasts are switching to Sculpsure for localized areas of fat. Ultherapy uses focused ultrasound energy to target the SMAS (Superficial Musculo-Aponeurotic System), which is the muscular tissue that is normally sutured or shrunk during a facelift. Unlike a facelift though, no incisions are required to reach this layer of tissue as the ultrasound will send directed energy waves to shrink it up. Easy enough and no recovery time required. A second pass is also made which targets the dermal collagen and forces the skin to generate healthy new collagen fibers to keep the skin firm. Although the procedure does hurt during treatment, there is no pain afterwards. Patients are able to tolerate and get through the procedure just fine. You can resume all normal daily activiities right away.<br />

ThermiRF is a monopolar radiofrequency device. First, the neck and jowl skin is numbed using a small blunt canula. The procedure is entirely pain and sensation free after the numbing. Next, a thin probe is inserted in a fan like pattern across the neck and jowls, while heating the area of the SMAS and skin. Temperatures of 60-65 degrees are reached under the skin to enable for tightening and shrinkage of the SMAS. If there is excess fat in the neck, there will also be some fat apoptosis for better contouring. The skin temperature target is 42-45 degrees for optimal collagen stimulation. After the procedure, a chin strap is worn for 48-72 hours to provide mild compression to the area. Swelling is seen for several days. There is minimal to no bruising afterwards. We recommend restricting activities for a few days or a week until the swelling subsides. ThermiRF certainly delivers the more dramatic results for patients with more significant skin laxity. It is the perfect procedure for someone requiring a facelift but does not want surgery. You are entirely awake and alert throughout this painless procedure.<br />

As we age (oh, that dreaded word), we sometimes notice that our eyebrows are beginning to droop, causing our eyelids to look heavier. What can we do to fix this and why does it happen? Similar to the drooping we experience in the lower face, jowls and neck, we see a loss of volume, specifically in the temples and above the brows, loosening of the fibromuscularature under the skin and loss of elasticity and collagen in the skin itself as being the prime factors. Correcting some or all of these factors can give very pleasing and natural results.<br />
<br />
Starting with the fibromusculature, there are several ways to achieve tightening of this layer. First and most dramatic is a surgical or endoscopic eyebrow lift. This is accomplished with an incision behind the hair line and lifting of the brow area much like a facelift. If one wishes to avoid incisions and scars, recovery time, and the risk of an "overdone" appearance, there are several excellent noninvasive, nonsurgical options. The best is Ultherapy. Ultherapy delivers focused ultrasound energy directly onto the fibromuscular layer to cause a contraction and tightening to occur in this structure without having to suture it surgically. The area treated is right above the eyebrow to the hair line. Furthermore, Ultrasound energy is focused also into the deep dermis where it stimulates new collagen and elastin fiber formation. Results are seen gradually over 4 to 6 months. Cost for an Ultherapy brow lift procedure is $1500. The lower eyelids can also be tightened for an additional charge. If there is concurrent loosening and drooping of the upper eyelid, I recommend using Thermismooth, Exilis elite or Thermage to tighten the eyelid skin. Ultrasound can not be used so close to the eye area. Thermismooth, Exilis elite and Thermage are all monopolar radio frequency procedures which heat the skin to a target temperature of 42-44 degrees, inducing new collagen and elastin fiber formation. The heat also likely affects the ligaments and fascia inducing a contraction within those tissues. These radiofrequency procedures range from $650-$3500 depending on the technology used. None of these procedures have any recovery time outside of mild redness and swelling from a few hours to a day. Make up and moisturizer can be worn immediately and all normal and strenuous activities can be resumed right away. Patients can return to work and social activities immediately. Possible side effects could include bruising, temporary sensation changes such as numbness or a possible skin burn. While very effective, results can be dramatic or subtle from patient to patient. Experience in administration and delivering the energy at the correct angle and temperature and time all work to give a superior experience and result for our patients.<br />
<br />
Next, we can use fillers. Fillers are made of natural safe ingredients such as sugar polymers (hyaluronic acid) or calcium particles. Some of the fillers you may have heard of include Juvederm, Juvederm Ultra, Juvaderm UltraPlus, Volume, Restylane, Perlane and Radiesse. I have not once seen an allergic reaction from fillers and allergy testing is not required. Care must be taken that the filler is not injected into a blood vessel or near a neurovascular bundle as it could cause occlusion to these structures. It's important to choose an experienced injector for this procedure. Placement is of greatest importance with fillers. The fillers will last anywhere from about 1 year to 2 years. For a longer lasting result, I will use Bellafilll in patients who have already experienced fillers and are ready for a more permanent result. To achieve a brow lift with a filler, I recommend first checking the temple area. We often see hollowing of this area--the temples begin to look concave versus the youthful convex shape. Adding volume back to the temples is very gratifying as it helps the brows and also helps to give a lift to the lower face. The second area to inject is right under and right above the eyebrow itself. This gives an instant lift to the eyebrow. There's a small chance of bruising, but can be easily covered with makeup if that does occur. Cost for filler is $500 and up per vial and several vials of filler may be required to get the right effect. We will often combine a noninvasive approach such as Ultherapy, Thermage, Thermismooth or Exilis elite with a filler to get immediate and best results.<br />
<br />
Another procedure I really like is called ThermiRase. ThermiRase is a unique procedure using a small probe in the temple area right under the skin via a tiny needle puncture. I can identify the nerve that causes the brows to move downwards towards the middle of the forehead. This occurs when one looks angry or is frowning or concentrating. We often Botox in this area to relax this tense expression and help soften the vertical lines that form between the brows called the elevens. With ThermiRase, the probe rests directly on the nerve that causes the contraction and zaps it with direct heat, disabling the nerve. If I can disable about 2 cm of this nerve, it rarely grows back and gives a permanent Botox like effect for the frown lines. Eliminating the frown expression gives an immediate lift to the eyebrows as the downward pull from the frown muscles is eliminated. If one is not ready for ThermiRase, of course Botox itself will also give a beautiful brow lift. Botox will last about 3-4 months, but may not be required as frequently with consistent use, as it relaxes the muscle and the muscle memory or tetany. Adding a little Botox to the RIGHT areas in the forehead and the lateral brow can give an amazing instant brow lift. The right areas is emphasized because if injected into the wrong muscle groups, it can do the opposite and cause the brows to droop. Fortunately, this very rare possibility completely reverses itself within a few weeks if it does occur. Again, an experienced injector is key.<br />
<br />
If one has wrinkles, extensive sun damage, and laxity of the skin, laser skin resurfacing works beautifully to rejuvenate the top layers of the skin and tighten the skin as well for a very dramatic beautiful result. Recovery time can be from 4-10 days varying on the technology used and depth of laser ablation. This is the only procedure with any significant recovery time issues. <br />
<br />
I hope this is helpful in sorting out some of the options for brow lift. I will mention but not recommend another procedure out there using threads under the skin to pull tissue up. I used to do these types of treatments many years ago, but do not offer them any longer as the threads are not strong enough to get any type of long lasting benefit. The problem is the thread may or not pierce and grab hold of the fibromuscular layer. Usually it only travels through the subcutaneous fat which does not grab and hold the suture well. With an exaggerated movement, it is easy to loosen the sutures under the skin and loose the intended effect. <br />

occurring in our skin and is hydrophilic (attracts water).<span style="mso-spacerun: yes;"> </span>This gives a good plumpness and longevity of
at least 1-2 years.<span style="mso-spacerun: yes;"> </span>We have found that
lip fillers tend to hold much longer than fillers applied to other parts of the
face.<span style="mso-spacerun: yes;"> </span>The same is true for under eye
fillers, which also tend to last longer than injections elsewhere on the face.<o:p></o:p></span></div>

appearance, and wish it would stay just so! After a few days to a week, the
swelling will settle down, revealing new plump lips.<span style="mso-spacerun: yes;"> </span>If more volume is desired, it can always be

starting with a minimal amount and building up gradually to the final desired

We have two methods of treatment. The first are the noninvasive treatments. These include Velashape, Thermage and Exilis
Elite. Velashape uses a combination of
suction, infrared head and radiofrequency energy to heat and knead the
skin. The idea is to achieve localized
fat apoptosis, to lengthen the fibrous septae and to stimulate collagen in the
skin. When performed diligently by an
experienced and dedicated clinician, the results are actually very good. Periodic maintenance after a series of
treatments is a good idea to keep the areas smooth. Thermage and Exilis Elite
both use monopolar radiofrequency to deeply heat the tissue to induce apoptosis
of the fat cells and stimulate collagen in the skin, essentially achieving very
similar results. Thermage is an
excellent treatment and can be accomplished in a single visit (though a repeat
treatment after 3-6 months would yield heightened results), but it is painful. Exilis Elite is performed in a series of 4
weekly treatments. Hydration is very
important with all these procedures as well hydrated skin allows for delivery
of higher temperatures through the skin for greater efficacy. <o:p></o:p></div>

The second method are the minimally invasive technologies
which include Cellulaze, VaserSmooth, and the latest ThermiTight. With all three procedures, the skin is first
numbed using a local anesthetic solution.
All of these procedures require wearing a compression garment immediately
following treatment for 1-2 weeks and all three follow a single treatment
protocol. With Cellulaze, a spagethetti
thin canula is threated back and forth through the cellulitic areas to 1) melt
and destroy fat cells, 2) pop and break through the fibrous septae and 3)
stimulate collagen via heat delievered directly under the skin. VaserSmooth works in an identical fashion,
but instead of laser energy, uses the gentler power of ultrasound to accomplish
the same three goals. The advantage of
VaserSmooth is less disruption to the vessels and nerves, where the laser
cauterizes through these structures. Therefore
the recovery time is a bit faster with less bruising and less swelling. The third method called Thermitight again
accomplishes all three end goals, but utilizes Radiopfrequency instead. The greatest advantage with Thermitight is
the strict temperature control of the device both internally and on the
skin. An infrared camera is constantly
watching the skin temperature to keep it in a safe therapeutic zone and the
tiny probe under the skin also allows delivery of exact therapeutic
temperatures to effectively destroy fat cells, break through the fibrous septae
and tighten the skin. Of all three
technologies, Thermitight yields the best skin tightening effects and
effectively treats the fat and septae as well.
These minimally invasive treatments are perhaps more permanent than the
noninvasive options. Recovery time is
typically one to two days with return to exercise after 2-3 weeks. <o:p></o:p></div>

Which method to choose can become a challenge, especially
when there are so many great options.
The first consideration would be whether a minimally invasive option is
acceptable for the patient. If so,

and there is no time for wearing a garment or recovery, the noninvasive options
are all great choices. The newest of the noninvasive procedures is
Exilis Elite. Even though it is a series
of treatments, patients prefer the price point and the comfort during treatment
(compared to Thermage which can be painful while being administered). <o:p></o:p></div>

There are now similar devices you can use at home with very short needles. The one we used had long needles that penetrated into the deep dermis and caused immediate pinpoint bleeding. It definitely worked and had benefits for skin laxity, scars (including stretchmarks) and wrinkles.<br />
<br />
Along came the fractional lasers and we found those a bit easier to use. Also, we could control the depth of penetration, number of spots per cm and could add coagulation to heat the skin for better shrinking and tightening ability. <br />
<br />
Today, CIT has made a comeback, almost like an article of clothing that goes out of fashion for a while and is reintroduced as something new and exciting. But the technology has changed too for the better. Gone are the rollers. In are the pens, of which there are many. Tiny needles, usually around 12 or so are attached to the end of the pen on a spring. They can penetrate anywhere from 0.5 mm to 3 mm into the skin delivering thousands of pinpoint sharp punctures into the skin per minute. Conceptually this is similar to the fractional lasers. However, there are some important differences to consider. First, there is no heat generated with the pins. Hence, the safety is better especially for our ethnic patients. There is no sloughing of the skin as there is no "ablation" or vaporization of any skin tissue--just the little tiny pinpoints. Hence, the recovery time is much shorter. <br />

All of our technologies have a place in our practice, and selecting the right procedure for each patient is our specialty. Many factors are taken into consideration when deciding which procedure to use including recovery time, skin color, lifestyle factors, the problem we need to improve and what it will respond best to, etc.<br />

Being in a position of having gained weight or inches, whether from poor lifestyle choices, genetics or medical problems, can be a very uncomfortable place to be in. Not only can it reduce self esteem and confidence, but it can also be depressing when your clothes don't suddenly fit correctly. Many of our patients also complain of just how uncomfortable those larger areas feel. The first steps to take are 1) Make sure there are no underlying medical conditions such as underactive thyroid that are reducing your metabolism. 2) Make sure you are following a healthy, balanced lifestyle by eating the right kinds of foods (avoid sugar, artificial sweeteners, all processed foods, hydrogenated oils, corn products, fat free foods, etc. Do consume mostly vegetables, try to go organic, and stick more to whole foods and seasonal fruits, etc.--this is really just a brief list). 3) Incorporate moderate exercise or activity into your daily life (if you can incorporate yoga and meditation, that will be great too). <br />
<br />
For many patients, even doing all the right things doesn't get rid of certain stubborn pockets or areas. For women, the belly, love handles, arms and thighs are the most frustrating areas. For men, the belly, love handles, chest and under the chin areas seem to be most popular. There are two procedural methods for removing excessive fat. <br />
<br />
The first category includes procedures which "poke holes" into the cell wall of the fat cells and allow the triglycerides, glycerine and fatty acids to spill out and be released from the body. These treatment methods require hydration, exercise and diet restriction to work. The latter three must be a continual effort to maintain results, otherwise, the hungry fat cells will begin reaccumulating fat and growing again. Procedures that fall into this category include i-lipo, Lipolaser, LiLa strawberry laser, and Zerona. Many of these are featured on groupon deals and fall into a fairly low price range. They are painless treatments using a cold red light laser beam that is applied via paddles or aiming beams over the treatment areas. Multiple weekly treatments are recommended. There is no recovery time and no pain with these treatments. Following strictly recommended water consumption, dietary restrictions and exercise recommendations will help these procedures be more successful because once that fat is released, it is essential to use it up right away before the body puts it back into storage. There have not been any documented risks. Perhaps the biggest risk is that it may not work for you. There are so many confusing names and technologies floating around. If you have heard of one and aren't really sure how it works, let us know and we'll be happy to check it out for you.<br />
<br />
The second category includes procedures which actually cause apoptosis or fat cell death. These procedures give a more permanent result. It is still important to continue following good lifestyle habits after these procedures in order to maintain the results. It is true that once an area experiences permanent loss of fat cells, it is not as easy to regain the fat in those areas. Regardless, it is still crucial to make sure you have good lifestyle habits in place that you can stay consistent with prior to and after such procedures. This category of procedures is far more exciting because the results are more dramatic. There are noninvasive treatments and then our very favorite Vaser Liposuction.<br />
<br />
Of the noninvasive Fat permanent fat cell killing procedures, here is the list:<br />
1) Liposonix: Uses High Intensity Focused Ultrasound (HIFU) to target fat cells under the skin and cause their cell death. The fat and cells are slowly eliminated over a 3 month time period. The procedure can be repeated at 3 months for improved results. This is a single treatment. There is some discomfort during the procedure which can be curbed with oral pain medications or with a little local tumescent numbing medicine placed in the deep fat layers. This procedure, as with all the nonsurgical permanent fat destroying methods, are better suited for patients in decent shape with small areas to loose. We have certainly been impressed with results we have achieved with this technology. Some patients have lost up to 5 inches from their abdomens. It also works well for arms and thighs.<br />
<br />
2) Vanquish: Vanquish uses the power of radiofrequency delivered through the skin via electromagnetic energy. Its prototype was first used in the Czech republic to treat muscle spasms, when it was discovered that these patients were also achieving permanent fat loss. This is currently among our most favorite procedures. There are three panels on this device, which allow for excellent coverage of the stomach and the sides at the same time. Some patients also have us treat their back fat and love handles. Besides destroying fat cells, this also helps to tighten the skin, so the best part is achieving excellent body contouring with this process. We love how it spread the energy very evenly over the entire treatment area. We might even combine this procedure with liposonix or exilis elite if there are particularly bulgier areas that need extra attention. The very best part is that this procedure is painless. It feels like a warm blanket over your skin. I was particularly surprised when I tried this procedure how hot the skin and underlying tissue actually feels to touch not causing any discomfort at all. Like liposonix, this procedure is fat selective and will not cause damage to any surrounding skin structures. We recommend one treatment a week for six weeks. You will see improvements occurring over 3-4 months after treatment.<br />
<br />
3) Exilis Elite: We primarily use this monopolar radiofrequency device for skin tightening and cellulite, but it is also excellent for spot fat reduction, where it will cause apoptosis to fat cells. In a way, this procedure is very similar to Thermage, but without the pain and with better control of treatment depth. It is also less expensive than Thermage. During this procedure, a wand emitting the radiofrequency energy is continuously pressed and moved over the troubled area(s) while we measure the heat we are generating under the skin. Like Vanquish, the radiofrequency energy is also going to stimulate new collagen growth for tighter skin as well as kill fat cells in the area. Apoptosis or cell death of the fat cells can be achieved if we can heat the fat to 45 degrees for 5 minutes continuously. We recommend one treatment a week for 4 weeks. More treatments can be done for even greater improvements if needed. No pain with this procedure either. Just a steady heating sensation.<br />
<br />
4) Coolsculpting: We do not actually offer this procedure but I wanted to mention it for the sake of being complete. With the advent of Vanquish, we don't feel Coolsculpting has much of a place in our practice or any advantages. It uses the concept of applying very cold temperatures to the fat cells, which destroys the fat cells but does not harm the skin. The skin and fat are suctioned into a handpiece, which is held over the area for one hour. Unless the practice has multiple devices in use, it takes about 3 hours if you wish to treat lower stomach and two love handles. When we tried this technology, the major complaint was the thawing process afterwards as being very uncomfortable for several days. Results didn't seem very impressive either. The other concern is that it can lead to lumpy areas or ridges where the technology grabs the skin. There is no doubt that this procedure works, but I feel we have much better options.<br />
<br />
So, now we move into my most favorite category which includes the Liposuction procedures. These are minimally invasive. Our patients are entirely awake and conversing with us or lost in their music throughout the treatment. A local tumescent numbing medicine is spread into the fat layers. With traditional liposuction, a suction canula with sharpened edges would next be inserted to manually suction out the fat cells. With our Vaser and Smartlipo technologies, we use the power of Ultrasound or Laser to first selective target and liquefy the fat cells for us. I am then able to gently suction the fat cells out with much smaller blunt suction cannulas. Overall, there is less trauma, less bruising, less swelling, less temporary nerve sensation loss, and back to regular activities much sooner. It does take a little longer than traditional liposuction, but the rewards are much greater. You will get up and walk home and return to work after two days. Also, because the ultrasound and laser energies help heat the skin, we can achieve much better tightening of the overlying skin compared to traditional liposuction. My preference these days is to use the Vaser technology as it is very selective for destroying the fat cells whereas the laser canula will cauterize or burn through vessels and nerves. The laser treatments also take much longer to complete in larger areas because the laser fiber is spaghetti thin. With Vaser I can reach the deep to superficial fatty areas easily and more completely. The ability to sculpt an area is amazing. The results are dramatic and artisitic. There is a compression garment we recommend wearing anywhere from two weeks to six weeks depending on how much fat we are removing. The most popular treatment area is the stomach and sides, followed by thighs, knees, calves, arms, bra fat, male breasts and chin/jowls. It can also be used under the eyes to gently remove puffiness or festoons. Also, if you want to transfer your fat to another area, this can easily be accomplished. The fat cells we collect via Vaser liposuction are far more viable and establish themselves easier than those removed via traditional liposuction because they are less traumatized. Many patients request enhacement of their breast and/or buttocks. When treating the chin/jowls areas, it can be an excellent intermediary step prior to requiring a facelift. <br />
